Have your say and help shape the new health and wellbeing ‘place’ plan

Cheshire West and Chester Council and local NHS are developing new proposal for residents’ health and wellbeing. The draft Cheshire West Place Plan outlines the borough’s approach to health and wellbeing over the next five years. It will replace the current Health and Wellbeing strategy.
Within Cheshire West the demand for health and care support services is increasing rapidly as our population ages and our local health challenges increase. There is a drive in the borough, as well as on a national scale, to integrate health and social care services, in order to improve population health, increase the quality of care and address increasing financial pressures on the health and care system.
The approach to integrating care is place-based with services being offered in a more joined-up way, improving efficiencies across the borough and enabling local people to access high-quality care when they need it. The aim is to prevent ill health, unnecessary hospital admissions and ensure that local, high quality services are sustainable for the future.
The five-year plan describes how Cheshire West will become an even better place to live well and enjoy life to the full. It also describes what residents can do themselves to stay healthy and well.
Councillor Louise Gittins, Leader of Cheshire West and Chester Council said, “We want everybody in Cheshire West to have a happy and healthy life. The plan sets out what organisations like the Council, the local NHS and the voluntary sector can do to support people to achieve this. Our vision is for people to have better physical and mental health and more years of healthy life.”
The Council and the NHS want to hear what you think of the draft plan. It is important that you have a chance to influence the final content.
