Live Well Cheshire West – Many people in Cheshire West and Chester need extra support to live full and enjoyable lives. You can find details about the support, advice and guidance available, across the area from a variety of sources, on the Council’s Live Well website. Here you can see clearly what services are available to you, and the people you care about, and how to access them.
The website can help with:
- support for carers
- advice about suspected abuse or neglect
- living safely at home
- staying independent
- identifying care needs
- paying for care and support
- choosing the right accommodation.
Cheshire West Integrated Care Partnership – Local NHS and local authority partners are moving towards integrating health and care services. By joining up services which are currently provided separately, they believe they can make better patient decisions by pooling experience, expertise and resources.
Integrated care provides the opportunity for partners to go further and faster to organise services in a more joined-up way and create a more efficient system which enables local people to access high-quality care when they need it.
On July 1st 2022 NHS Cheshire and Merseyside replaced the clinical commissioning group infrastructure, with nine borough ‘Places’ tasked with working to improve health and care outcomes under the umbrella of the new organisation – a statutory Integrated Care Board.
There are also two main Hospital Trusts:
- Mid Cheshire Hospitals NHS Foundation Trust – consisting of Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst in Winsford.
- Countess of Chester NHS Foundation Trust – consisting of the Countess of Chester Hospital and Ellesmere Port Hospital.
Mental Health, substance misuse, learning disability and community physical health services are provided by Cheshire and Wirral Partnership NHS Foundation Trust.
